2021-22 School Year: Grades 1st through 12th
All families must provide proof of residency (step 1 below ) and complete the online registration (step 2 below) prior to students attending school.
Please follow the 3 step instructions below if you meet the following criteria.
- You have no children currently in the North Polk Community School District.

Step 1: Proof of Residency
- Complete the Proof of Residency Form 2021-22.
Step 2: Online Registration
COMPLETE STEP 1 FIRST ABOVE.
- For families New to the District: Complete the online registration by clicking this link here.
If you are an existing family - and need to add a new student not already on Infinite Campus. Log into your parent portal here. Go to More on the bottom left and then select Online Registration. (there is an Add Student button to add your new student in the Online Registration).
Once approved, you will be required to show the certified birth certificate of your child and updated immunization records to the school secretary. The State Health Requirements checklist is provided over on the right hand side. You will be sent an email when your online application is approved and will receive a separate email shortly after with a user name and password to log in to the Parent Portal.
The Parent Portal is a website that can be used to view your child’s school information (schedules and grades), as well as pay for school fees and lunches. Step 3: Health/Immunzation Forms
- Please send any forms (including transportation or health forms) that you printed or completed during the registration process to the secretaries of the school your student will be attending.
