IMPORTANT INFORMATION FOR FAMILIES OF STUDENTS ENTERING 7th GRADE IN THE 2019-2020 SCHOOL YEAR
All students entering or transferring into 7th grade and born after September 15th, 2004 will need proof of one dose of meningitis vaccine on or after their 10th birthday. They will also need proof of a Tdap vaccine given after 7 years of age. Call your doctor’s office now to make your child’s appointment for these vaccines. Do not wait, as appointments will fill up fast and your child may not be able to see their provider in time. If you do not have health insurance or have a doctor, please call the Polk County Health Dept. at (515) 286-3798. Your child can qualify for the Vaccines for Children Program and can receive the vaccines at little cost. If you have additional questions, please contact me at the Middle School. Angie McGough, RN BSN 515-984-3400 ext 2221 or firstname.lastname@example.org.
REGISTRATION FOR THE 2019 - 2020 SCHOOL YEAR
It is that time of year again! We are needing all North Polk families to register their students for the next school year (2019-20). Registration is important for many reasons, but primarily because it provides an opportunity to review and update household information each year. In the event of an emergency, it's important to have accurate contact information and medical records on file. For students in grades 7-12 that participate in extra-curricular activities, the process fulfills Heads Up Concussion requirements and includes Code of Conduct information required for participation.
With that said, we need all families to register! You can do this online by logging into your Infinite Campus Parent Portal. Once you're logged in, select the "More" button in the bottom left, then scroll down to "Online Registration" and select "Click here to go to Existing Student Registration." For those of you with students entering NP for the first time, you will have the opportunity to add additional students to your household's existing account. Please help us achieve 100% registration for returning families this year!
With questions or help logging into your Parent Portal, please contact Lynette Foster at email@example.com.
