New Student Registration

Welcome to the North Polk Community School District!

To register your new student for the 2024-2025 school year, please follow the TWO steps outlined below:

• For students entering Kindergarten or Transitional Kindergarten, please ensure your child is 5 years old on or before 09/15/2024.

• In-District Families: This step is essential for ALL incoming TK/KG students (even if they are already in our system) and ALL new students to the district. Please complete Step 1 BEFORE reviewing Step 2 to help us gather accurate numbers for our elementary classrooms.

COMPLETE STEP 1 AND 2 BELOW - YOU WILL NOT BE APPROVED UNTIL BOTH ARE COMPLETED

Requirements for Step 1:

Please have the following documents ready for upload:

• Proof of Residency (Acceptable proof of residency may include a current lease, property tax notice, homeowner’s insurance bill, mortgage statement, current vehicle registration form, any utility bill listing your residence as the service address, a letter from a shelter, or a letter from your employer, if your employer provides housing.)

• Proof of Student Age (e.g., birth certificate, hospital certificate of live birth, US passport, adoption papers, custody document, or student's driver's license)

Step 1: Complete the Sign-Up Google form here. All new students, INCLUDING incoming TK/KG students, must complete Step 1 to verify their proof of residency/proof of age for each student. Failure to complete this form will result in your student not being placed on a roster. (Please do not add existing 1st through 12th graders here; existing student registration can be completed here).  

Step 2: Parent Portal Information

• New Families (no kids in the district yet): If you are a new family with no children in the district, please complete the online registration portal here. This portal will capture your contact information, permissions, and child's information and will take approximately 10 minutes to complete. Please use a computer for this process. Once your online registration is approved, you will receive a username and password for the parent portal.

• Existing Families: If you are an existing family and need to add a new student who is not already in Infinite Campus, log into your parent portal here. Go to "More" on the bottom left and then select "Online Registration." Please register your EXISTING student first and save the information, and then click the  "Add Student" button to include your new student in the online registration. Save your new student information. Once all students have been reviewed, click Submit. *You will need to register all students in the parent portal to request busing for the fall of 2024-25.

Step 3: Required Forms

• Please submit any forms, including transportation or health forms, that you printed or completed during the registration process to the secretaries of the school your student will be attending on Back-to-School/Meet-the-Teacher night in August.

• Immunization forms will be required by the nurse's office before your student's first day of school.

Step 4: Fee Payment
Fees will be assigned on July 15th, 2024. Log in to your parent portal to pay your class and textbook fees. Payment is due before your student's first day of school.
