New Student Registration

STEP 1: Complete the Sign-Up Google form here. Failure to complete this form will result in your student not being placed on a roster. Please have the following documents ready for upload:

Please use a computer/laptop for step 1 or 2 below. A phone does not work!

• Proof of Residency (Acceptable proof may include a current lease, property tax notice, homeowner’s insurance bill, mortgage statement, current vehicle registration form, any utility bill listing your residence as the service address, a letter from a shelter, or a letter from your employer if they provide housing.)
• Proof of Student Age (adoption record, certified statement of a physician, passport, hospital record of live birth, custody document, an official document from another country, immunization form with a birth date, or student's driver's license.)

This portal will capture your contact information, permissions, and child's information and should take about 10 minutes to complete. For the best experience, please use a computer. Once your online registration is approved, you’ll receive a username and password for the portal.

Complete Registration Step 2

Fees will be assigned around August 1st. Please wait for your activation code, then log in to the parent portal to pay your class and textbook fees. Payment must be completed before your student's first day of school.

Please submit any forms, including transportation or health forms, that you completed or printed during the registration process to the secretaries at your student's school on Back-to-School/Meet-the-Teacher night in August.

Additionally, immunization forms must be submitted to the nurse's office before your student's first day of school.

Registration for the 2025-26 school year opens on February 10, 2025. We encourage families to register by June 1, 2025 to ensure transportation requests are processed and to guarantee busing for the start of the school year.

You will need to submit one of the following as proof of residency

Yes, the district is currently accepting open enrollment applications for families residing outside the district boundaries. Applications are being accepted until March 1, 2025. Please note that while applications will be accepted, they are not guaranteed. For out-of-district families wishing to enroll at North Polk, please contact the District Registrar to inquire about open enrollment availability.

Yes, you can request transportation during the New Student Registration process. When completing the registration in the Parent Portal, there will be an option to select transportation.
